I'm interested! What are the next steps?
Please contact us. We’d love to get to know you better, answer any questions you might have, and introduce you to a few members of our team. If you have additional questions beyond these post-acute care facility FAQs, our team is happy to help.
What insurances do you accept?
We are proud to accept private pay, all PPOs, and a wide range of HMOs including Kaiser, John Muir Physicians Network (JMPN), Medicare, Medicaid, Tricare, and more. No matter what type of insurance you have, our insurance experts will take time to help you understand your plan benefits and typical coverage for doctor visits in post acute care as well as services coordinated with outpatient care centers following discharge.
Do you offer short-term rehabilitation?
Yes. At Rossmoor, your successful recovery and quick return home is our top priority. We provide a personalized experience whether you’re recovering from an illness or a major medical event. Physical, occupational, and speech therapy is conducted under the guidance of licensed experts and is available seven days a week. When appropriate, we coordinate a smooth transition to outpatient care centers so you can continue therapy after discharge.
What is your visitor policy?
At Rossmoor Post Acute, we believe that family and friends play a critical role in our residents’ emotional and physical well-being. Family involvement in post acute care is encouraged throughout your stay. Contact us for the most current, detailed information regarding available visitation options and guidelines that support meaningful family involvement in post acute care.
When will the doctor see me?
Doctors typically visit within 24 hours of admission. Your nurse will communicate any medical concerns with your doctor before and after they see you. As part of doctor visits in post acute care, our clinicians coordinate closely with your hospital providers and, when preparing for discharge, with outpatient care centers and your primary care physician to ensure continuity.
How often will I have therapy?
Most patients, depending on their condition and when they are admitted, will start rehab within 24 hours of admission and will receive therapy up to five times per week. After discharge, many patients continue therapy at outpatient care centers to maintain progress and reach long-term goals.
How long is a typical stay?
Each case is different and unique. Patient progress is reviewed weekly by our Interdisciplinary Team (IDT) of nurses, therapists, and dietitians. Our main goal is that you reach your highest level of independence. These reviews, similar to the clarity provided in our post-acute care facility FAQs, guide your care plan, timing of doctor visits in post acute care, and the transition plan to home or outpatient care centers.
Are activities available?
We offer daily activities for a wide variety of interests. For your convenience, there is an activity calendar posted in your room. Please contact the Activities Director for additional information. We also welcome family involvement in post acute care activities whenever possible to support social connection and recovery.
What is your dining program like?
At Rossmoor, we’re proud to offer restaurant-style dining with diets designed by our full-time, in-house Registered Dietitian. Menus can be tailored to your clinical needs and preferences to complement your therapy and recovery goals.
Do I need to bring in my medications?
No, you do not need to provide medications unless specifically told to do so. Upon admission, we will order your medications using our off-site pharmacy, then securely store them on-site to ensure safety. Medication reconciliation is coordinated with your physicians during doctor visits in post acute care and again when transitioning to outpatient care centers or home.
What type of clothing items should I bring?
We encourage you to bring a pair of comfortable shoes and at least seven days’ worth of comfortable clothing including pajamas and undergarments. Clothing that is easy to put on and take off is ideal for therapy sessions.
Can I bring personal belongings?
Yes! We encourage you to bring personal items such as family photos, blankets, and decorative items—anything that will help make your stay more comfortable and personalized.
How do you protect patients, staff, and visitors from COVID-19 and other infectious diseases?
Our team is highly trained and experienced in caring for high-acuity patients and infectious diseases like COVID-19. Our dedicated infection prevention nurse helps ensure we follow current infection prevention guidelines and best practices. This includes educating all staff about the appropriate use of personal protective equipment (PPE) such as N95 masks, surgical masks, gloves, and gowns and when to use COVID-19 protocols required by the local department of public health and/or the CDC. These safeguards apply to clinical care, therapy, and doctor visits in post acute care, as well as to safe coordination with outpatient care centers.
